TB Blood Test or Chest X-Ray? Which One You Actually Need

For a UK visa, a TB blood test is not accepted. The Home Office requires a chest X-ray at one of its approved clinics overseas. QuantiFERON blood tests are used for latent TB screening in occupational health, before certain treatments, and for some non-UK immigration processes.

Will a TB blood test satisfy a UK visa requirement? 

No. The UK Tuberculosis Technical Instructions are explicit: a chest X-ray is the only acceptable screening tool for active pulmonary TB for visa purposes. Tests for latent TB, including QuantiFERON-TB Gold and T-SPOT.TB, are not accepted as alternatives, even where the applicant offers to pay for them privately. 

The X-ray also has to be taken at a Home Office-approved clinic in the country you are applying from. A certificate from any other clinic is rejected, however reputable that clinic is. If the X-ray is unclear, a sputum sample may be requested, and that can add weeks. 

We are stating this plainly because several UK providers advertise QuantiFERON testing for visa applications. If your paperwork is a UK visa, that is not the test you need, and buying it will not move your application forward. 

So what is a TB blood test for? 

QuantiFERON-TB Gold is an interferon-gamma release assay, or IGRA. It measures how your immune system responds to proteins associated with tuberculosis bacteria. It is used to assess latent TB infection, which is TB the body is containing without causing symptoms. 

In the UK it is routinely used for occupational health screening, particularly in healthcare roles; for contact tracing after exposure; and before starting certain immune-suppressing treatments. Some overseas visa, work permit, university and residency processes also ask for an IGRA specifically. 

How is it different from the skin test? 

The tuberculin skin test involves an injection and a return visit to read the result, and it can be affected by previous BCG vaccination. An IGRA is a single blood sample, needs no return appointment to be read, and is not usually affected by prior BCG. 

How do I know which one I need? 

Read the exact wording of your requirement. If it names QuantiFERON, IGRA, or a TB blood test, a blood sample is what is being asked for. If it refers to a chest X-ray, an immigration medical examination, or a panel physician, a blood test will not substitute. 

When the wording is ambiguous, go back to whoever asked for it before booking anything. A wrong test is a wasted fee and, worse, lost time on a deadline.
