WHEN A BLOOD TEST HELPS
Do you need a blood test for menopause? It depends on your age.
During perimenopause hormone levels swing from day to day, so a single result can’t confirm or rule out the menopause. These age bands come from the NICE menopause guideline (NG23).
Under 40
Testing is needed
Two FSH blood tests, 4 to 6 weeks apart, to check for premature ovarian insufficiency.
40 to 45
A test may help
An FSH level can be considered alongside your symptoms, particularly if your periods have changed.
Over 45
No hormone test needed
With typical symptoms, perimenopause and menopause are diagnosed from your symptoms.

What is the menopause — and perimenopause?
“Menopause is a normal stage of life, not an illness. But the symptoms are real, they can last for years — and they are treatable.”
Menopause is when your periods stop for good, because the ovaries have stopped releasing eggs and making most of their oestrogen. The average age in the UK is 51. The main medical treatment for menopause symptoms is hormone replacement therapy (HRT).
No unnecessary hormone tests
If you’re over 45 with typical symptoms, NICE guidance says you don’t need a hormone test to be diagnosed. We’d rather spend that time listening to you.
Perimenopause is the run-up — often several years — when hormone levels rise and fall unpredictably. Symptoms usually start here, while you are still having periods.
Menopause before 45 is called early menopause, and before 40 premature ovarian insufficiency. Both are assessed differently, and treatment is usually recommended at least until the natural age of menopause, to protect your bones and heart.

When HRT isn't the right treatment
For most women, the benefits of HRT outweigh the risks. Not for everyone — and we'll tell you when. If that's what we find, you get a clear explanation and a route forward: a non-hormonal option, a referral, or a letter to your GP.
We won't start HRT if:
You have unexplained vaginal bleeding — that needs investigating first, and we'll make sure it is.
You have, or have had, breast cancer or another hormone-sensitive cancer — that decision belongs with your specialist team.
You have a current blood clot, active liver disease, or could be pregnant. Some women with past clots or migraine can still have HRT in a form that suits them, with the right advice.
Something else explains it — thyroid problems, iron deficiency, diabetes, depression and some medicines cause many of the same symptoms.

Blood tests — only where they help
Under 45, FSH supports the diagnosis. For anyone, thyroid, full blood count, ferritin, HbA1c, liver, kidney and cholesterol can rule other causes in or out. Taken from a vein at a clinic near you — and not repeated if you’ve had them recently.

Consultation with the clinician
Your symptoms, history and any results, gone through properly — in the room at Crewe, or by video wherever you are. It may or may not lead to a prescription; that decision is made together.

Treatment, if it’s appropriate
If HRT is clinically appropriate, your clinician explains the options and the benefits and risks for you, including protecting the womb if you have one. Dispensed and posted by our partner pharmacy, and charged by us when your prescription is issued. HRT is not contraception, so we’ll make sure that’s covered too.

Reviews at 3 months, then every year
We look at your symptoms, bleeding pattern, blood pressure and side effects, and adjust the dose, type or route on the evidence. Most HRT doesn’t need routine blood tests — we test when there’s a reason.
